Application Guidelines

Applicants should submit two copies of a written proposal, including the following:

  • A cover letter from an official of the organization stating a brief description of the purpose of the project, the organization's formal approval of the proposed project or program, and the specific amount requested.
  • A full description of the organization including history, purpose, programs and services, audiences served.
  • A concise description of the need(s), how the needs were determined, the project or program solution, timeline, personnel and resources assigned, the unique difference of your project in the community, collaborators (if any), and the impact or benefits from addressing the need.
  • A detailed plan for funding the project including the total cost, the amount requested, funds raised to date or to be raised and a forecast of operating costs and method of funding.
  • A description of how the project or program will be evaluated or the impact measured.
  • A list of the governing board and officers.
  • One copy of the following attachments:  IRS forms certifying the organization is exempt under Section (501)C3 and is not classified as a private foundation as defined in Section 509(a), and the audited financial statements from two prior years.

All of the above must be included before the proposal can be evaluated.
